Please read the following case study and in an essay respond to the points that follow.
Sihle is a 20-year-old African man who is in his second year of university. He has arrived with
a close friend. He reports he is seeking treatment as he often hears voices (who he thinks are ancestors) that instruct him to do “bad” things, such as hurt and kill others, and reported that he has heard these voices over the last 6 months. He further reports that he feels his flatmate in his residence has a grudge against him and is “watching” him, causing him to feel more and more afraid to leave his room. His intense anxiety about this has begun getting in the way of his ability to complete schoolwork, and his friends are concerned – he says they have told him, “You’re not making sense.”. This makes him very angry and at times has become
quite aggressive in response to these comments.
Furthermore, his friend contests that his
flatmate in residence has any obvious grudge against him, but notes he is concerned about
Sihle’s refusal to engage in self-care (with a notable deterioration in his personal hygiene).
During your consultation, Sihle occasionally laughs abruptly and inappropriately and
sometimes stops speaking mid-sentence, looking off in the distance as though he sees or
hears something. His beliefs are fixed, and if they are challenged, his tone becomes hostile.
In getting collateral information from his family, who are from a rural community, and who
hold traditional religious beliefs, it is further reported that they were unable to conduct a
specific customary ceremony due to financial constraints, which they feel is significant in his
presentation of symptoms.
In an essay format, please address the following points in your response to the case study.
Please ensure that your essay provides an in-depth analysis of each of the points and
demonstrates a clear understanding of the concepts involved. Use credible sources to
support your arguments and cite them appropriately.
1. Primary Diagnosis: Using the DSM-5-TR criteria, provide your primary diagnosis of
Sihle. Please refer to the American Psychiatric Association website for DSM-5-TR
diagnostic criteria, as well as your prescribed reading material. Please ensure that
your response is not in point form.
2. Challenges of Diagnosis: Provide a critical review of the particular challenges of
diagnosing and working with the identified primary diagnosis in South Africa.
Substantiate your critical review with relevant, contemporary research and literature.
3. Management of the Case: The family in this case study present with divergent belief
systems. Using this as a basis for your discussion, consider up how you as a counsellor
would manage the case on an individual level (Sihle) and family level (Sihle and his
family). Include a critical discussion on how you would include a western and
traditional worldview in managing Sihle.

I think the question is asking how we, coming from a more western perspective, would manage the case considering the more traditional perspective they hold. ]
4. Psychoeducational pamphlet brochure: Design a psychoeducational pamphlet
brochure on the primary diagnosis that you identified in the case study. The pamphlet
should have 3 columns on 2 pages and should be creative, visually pleasing and user-
friendly for people from all walks of life in South Africa. You may make use of an app
such Canva Templates (canva.com). The pamphlet brochure should be written in
language that is accessible to clients at a local clinic. Information included should be
accurate and may include issues such as description of the illness; symptoms; myths
attached to the illness; ideas regarding support for the client and family; where to get
help; referral sources, and so forth.
