Add one for onboarding based on the feedback you read, and one for someone who has been here longer, at least 5 years. There is no limit on work count but utilize your past research and resources.
Make these corrections and add more questions:
For the onboarding one I would review and add/edit questions that are more focused on the new employee’s individual experience and feeling welcomed and included, rather than more broadly organizational feedback.
The next set for 5 plus years- I would try to adjust and use less “leaving language” but also why they stay, etc. we could use those quotes.
